Sleep deprivation is endemic in SOF populations and directly degrades cognitive function, emotional regulation, and physical recovery. The following studies demonstrate HBOT’s measurable impact on sleep outcomes across multiple populations.

HBOT Improves Neurocognitive Functions and Symptoms of Post-COVID Condition: Randomized Controlled Trial 

Zilberman-Itskovich S, Catalogna M, Sasson E, et al. (2022). Scientific Reports (119 citations) 

Sham-controlled, double-blind RCT (n=73). 40 HBOT sessions produced significant improvements in sleep (d=0.48, p=0.042) alongside cognitive function, energy, psychiatric symptoms, and pain interference. Brain MRI confirmed perfusion and microstructural improvements in regions governing sleep regulation and cognition. 

Long-Term Outcomes of HBOT in Post-COVID Condition: Longitudinal Follow-Up of a Randomized Controlled Trial 

Hadanny A, Catalogna M, Yaniv S, et al. (2024). Scientific Reports (39 citations) 

One-year follow-up (n=31) of the above RCT. Sleep improvements observed across five domains persisted at long-term assessment with moderate-to-large effect sizes (0.47–0.79). Pain, psychiatric, and quality-of-life improvements also maintained. Key finding: HBOT’s sleep benefits are durable, not transient. 

HBOT for Chronic Insomnia at High Altitude: A Prospective, Randomized, Open-Label, Parallel-Group Trial 

Sun B, Li Y, Wang Z, et al. (2025). Travel Medicine and Infectious Disease 

RCT (n=80, median age 25). 10-day HBOT protocol for chronic insomnia at high altitude. Sleep quality (PSQI) significantly improved in HBOT group (4.6 vs. 9.1, p<0.0001). Insomnia severity likewise improved (5.0 vs. 9.8, p<0.0001). Secondary improvements in fatigue, anxiety, and depression. No serious adverse events. Directly relevant to SOF personnel operating at altitude.

Synthesis: HBOT improves objectively measured sleep outcomes across multiple populations and study designs. A double-blind RCT shows improvements persisting at one-year follow-up. A high-altitude insomnia trial is directly applicable to SOF operational environments. Meta-analytic data confirms improvements in the sleep stages most critical for cognitive consolidation and physical recovery: slow-wave sleep and REM.
